Getting braces with us is easy. We offer our patients the option of either traditional metal braces, clear braces, gold braces, or Invisalign.
There are pros and cons to all of these choices, which is why we invite you to learn more about our different options.

We encourage all new patients to complete our patient information and health history forms above. Please print out each form, complete each, and bring the completed forms with you to your first visit to our office. These forms will be part of your registration process during your first visit. When filling out your New Patient Forms, be sure to include everything you know about your past and present state of health, any medications you are taking, and any diseases or disorders you have. All this information, in combination with the information we gather during your initial examination, will guide us as we develop an appropriate and effective treatment plan for you.
